A light bleach solution will kill anything. Let it soak for a good while then wash out. You may have to rinse a lot to get the bleach smell out but it will be sanitized.

Two options:
1. Buy a new reservoir - about $24, depending on the size: tinyurl.com/2gftkg
2. Buy a cleaning kit - about $20 (but labor-intensive): tinyurl.com/2bd422
Ebay may have these cheaper. If it's really gunked and more than 2 years old, just get a new one.
Some people have had success just using hot water with 2-3 drops of bleach, followed by a thorough rinse. Others use denture cleaning tablets (same thing as the tabs in the cleaning kit, really). A regular bottle-cleaning brush should get the main body clean, but you'll need the wire pipe-cleaner to really get the crud out of the tube.
I always use a light bleach solution like troo said. It works well, kills anything, and just needs to be rinsed and dried out. Really cheap, too. Just dont use a lot of bleach!!!
Bleach will kill all the nasty little buggies, if it still has a little bit of a musty smell, try soaking it with some vinegar (the white stuff is fine). Sometimes the odors are acid soluble.

A good way to make sure that the mildew doesn't come back, would be to leave it in the freezer when you're done. When the little bit of remaining water turns to ice, mildew will not grow. If you want a new one, Walmart sells 2 liter ones for $9.

B - Bright is a non-bleach disinfectant for food and beverage containers. It doesn't leave any taste behind. If you can't find this product rinse with a baking soda solution after using the bleach. That gets rid of any left over residue.
