I’m down for adding a new role, but I’m not a huge fan of the doctor. One of the things I love about our rules is it’s a lot more “intellectual” than classic mafia, and I feel that the doctor is not a dynamic enough role. You can really only play doctor two ways, super random, or by doing the same thing every time (trying to heal inspector). I’m gonna look through a full roles list and see if there’s anything that catches my fancy.
We view the doctor differently then. I think the doctor adds an element because of the mafia's need to get two people eliminated. The doctor also sets up a classic prisoner's dilemma a bit. It could encourage an inspector to come out earlier, like D2, hoping the doctor can save them. It could encourage the doctor to come out on D1 if they are in trouble. It would take a different read of the game from the doctor to tell who needs to be saved. We'd have to flesh out the order of operations and rules for it, but I think it's worth a shot. It's the easiest thing to add.
I’m also against role reveals I feel like it takes away an important layer of the game.
Definitely on board with this, for now.
I love our game here and don't think it needs wholesale changes. I think games can go more quickly, but if a ref wants to change the timelines I think that has to be noted when all players are signing up. Note that you are signing up for a game that is going to go more quickly, rounds will end on "weekends", etc.
It makes sense to me to do 18 hour rounds with 6 hour nights. Auto extensions of two hours for vote switches within an hour of the deadline.

You bring up some good points I hadn’t considered about the doctor. There is definitely going to have to be some meta game shifts that could play out pretty interesting. I’m really down to try any new role, there’s probably a couple cool ones I’ve never even heard of.
I’m indifferent about runoffs as well. I think the current way we have may be very slightly beneficial to town because it forces mafia to be more shifty with their strategy. However, there is the flip side with the current system where “I have a town read on both so this is random” is an acceptable reason for a vote. So I can be convinced either way.

If I had my way, we'd add doctor next game, and have tighter timeframes but no other rules about runoffs and such. If a player doesn't want runoffs, it gives them incentive to move their vote.
I think also, something I mentioned earlier, it would be interesting to have the ref inform mafia members individually at the beginning of the game, and then once D1 is over to create the group thread so they could start strategizing.
wait how does the doctor work? Could Inspector just reveal Day 1 then the doctor just keeps them alive through all 4 nights?

I think the rule is usually that the doctor can only save each player once. We'd also have to work out the order of operations. For example:
Scenario 1 - inspector comes out, doctor saves inspector, mafia whacks inspector - inspector lives, no mafia whack Scenario 2 - inspector comes out, doctor saves inspector, mafia whacks doctor (lucky guess) - inspector lives, mafia whack of doctor (no role reveal) Scenario 3 - mafia whack player X, doctor saves player X - player X lives, no mafia whack
So what I'm saying is mafia action happens first. Now that I'm thinking about it, I can't think of a way this wouldn't work out, but I'm sure there is one.

(Apologies... once upon a time, this image could be zoomed in more)... This is the spreadsheet Fawn and I used for Mafia 100 with doctor & vigilante. Maybe it's helpful for some of your ideas, maybe not, but it worked with that mess.

I will ref next game. We have four spots remaining, though piggy pablo wasn't clear whether he was in for next. He gets a spot (and slotted in appropriately) if he was saying he was in when he said he wanted to change things up.
As far as changes for the next game, here's what I am proposing (someone needs to take charge):
- add the doctor role - Compressed timeline - 18 hour days, runoffs allowed (town discretion), 6 hour nights (adjusted for time, I'm not posting at 4am) - auto extensions on vote changes within an hour of the deadline (2 hour extension automatically) - other extensions at ref's discretion (and I will reject most of them) - rounds ending on weekends is permissible
If there's consensus on another rule change, let's talk about it. Otherwise you are agreeing to these conditions.

Re: runoffs
Many times we get to a point where 4 players have 2 votes. We look at each other and say “are we cool with this runoff?” Everyone is. Then we just wait for the round to end and at that point we’ve added 24 hours to the game. Really, at that moment, the 3 people with future revotes can just progress the game by “switching” / “revoting” right then, getting it to 3 and saving a full day. And that should not be viewed as suspicious whatsoever. It should be viewed as someone being active and considerate of speeding up the game.

I would make the two-hour auto-extension conditional on time of day, depending on OG deadline time. Like if the OG deadline is 11pm/12am, pushing it another two hours might not be feasible for some people. Might need more than two hours.

This has been debated ad nauseum in the rules thread. I don't want to change it in a game I ref without consensus.
